Facts about Enlarged Prostate:

A man's prostate gland usually starts to enlarge after he reaches 40 years of age. This condition is called benign prostatic hyperplasia (BPH).

The prostate gland secretes a fluid that helps to nourish sperm. The gland itself surrounds the urethra, which is the tube that carries urine from the bladder out through the tip of the penis (see multimedia file 1). As the prostate grows larger, it may press on the urethra. This narrowing of the urethra can cause some men with prostate enlargement to have trouble with urination. Prostate enlargement may be the most common health problem in men older than 60 years of age.

According to the National Kidney and Urological Disease Information Clearinghouse, the most common prostate problem for men over 50 is prostate enlargement. By age 60, over one-half of men have BPH; by age 85, the number climbs to 90%, according to the American Urological Association (AUA).

What are the causes of Enlarged Prostate?

The actual cause of prostate enlargement is unknown. It is believed that factors linked to aging and the testicles themselves may play a role in the growth of the gland. Men who have had their testicles removed at a young age (for example, as a result of testicular cancer) do not develop BPH.

Similarly, if the testicles are removed after a man develops BPH, the prostate begins to shrink in size.

If the prostate grows larger and the urethra is squeezed more tightly, the bladder might not be able to fully compensate for the problem and completely empty. In some cases, blockage from prostate enlargement may cause repeated urinary tract infections and gradually result in bladder or kidney damage. It may also cause a sudden inability to urinate (acute urinary retention) -- a medical emergency.

Some facts about prostate enlargement:

  • The likelihood of developing an enlarged prostate increases with age.
  • BPH is so common that it has been said, that all men will have an enlarged prostate if they live long enough.
  • A small amount of prostate enlargement is present in many men over age 40 and more than 90% of men over age 80.
  • No risk factors have been identified other than having normally functioning testicles.

    What are the symptoms of Enlarged Prostate?

    Symptoms of enlarged prostate can include:

  • A weak or slow urinary stream
  • A feeling of incomplete bladder emptying
  • Difficulty starting urination
  • Frequent urination
  • Urgency to urinate
  • Getting up frequently at night to urinate
  • A urinary stream that starts and stops
  • Straining to urinate
  • Continued dribbling of urine
  • Returning to urinate again minutes after finishing
  • When the bladder does not empty completely, you become at risk for developing urinary tract infections. Other serious problems can also develop over time, including bladder stones, blood in the urine, incontinence, and acute urinary retention (an inability to urinate). A sudden and complete inability to urinate is a medical emergency; you should see your doctor immediately. In rare cases, bladder and/or kidney damage can develop from BPH.

    What treatments are available for Enlarged Prostate?

    Benign prostatic hyperplasia (BPH) cannot be cured, so treatment focuses on reducing your symptoms. Treatment is based on how severe your symptoms are, how much they bother you, and whether you have complications.

    Deciding how to treat BPH is greatly influenced by how bothersome your symptoms are.

    Symptoms that are mild or that do not bother you may be best treated by watchful waiting. This means you may make small changes to your lifestyle to control your symptoms, but you do not take medicines or have surgery. You have regular checkups to be certain your symptoms are not getting worse.

    Medications:

  • Finasteride and dutasteride lower levels of hormones produced by the prostate, reduce the size of the prostate gland, increase urine flow rate, and decrease symptoms of BPH. It may take 3 to 6 months before you notice a significant improvement in your symptoms. Potential side effects related to use of finasteride and dutasteride include decreased sex drive and impotence.
     
  • Alpha 1-blockers (doxazosin, prazosin, tamsulosin, terazosin, and afluzosin) are a class of medications also used to treat high blood pressure. These medications relax the muscles of the bladder neck, allowing easier urination. Two-thirds of the people treated with alpha 1-blocker medications report an improvement in symptoms.
     
  • Antibiotics may be prescribed to treat chronic prostatitis (inflammation of the prostate), which may accompany BPH. Some men note relief of their BPH symptoms after a course of antibiotics.
  • Saw palmetto has been used by millions of men to ease BPH symptoms and is often recommended as an alternative to medication. Experts suggest a fat-soluble saw palmetto extract with 85 - 95% fatty acids and sterols.

    Surgery - The choice of a specific surgical procedure is usually based on the severity of your symptoms and the size and shape of your prostate gland.

  • Transurethral resection of the prostate (TURP): This is the most common surgical treatment for BPH. The TURP is performed by inserting a scope through the penis and removing the prostate piece by piece.
     
  • Transurethral incision of the prostate (TUIP): This procedure is similar to TURP, but is usually performed in men who have a relatively small prostate. This procedure is usually performed as an outpatient without need for a hospital stay. Like the TURP, a scope is inserted through the penis until the prostate is reached. Then, rather than removal of the prostate, a small incision is made in the prostatic tissue to enlarge the opening of the urethra and bladder outlet.
     
  • Open prostatectomy: An open prostatectomy is usually performed using general or spinal anesthesia. An incision is made through the abdomen or perineal area (i.e., through the pelvic floor, including the region from the scrotum to the anus). Then, the prostate is removed. This is a lengthy procedure, and it usually requires a hospital stay of 5 to 10 days.
