Facts about Heart Disease:
Heart disease is
a number of abnormal conditions affecting the heart and the blood
vessels in the heart. The patient suffering from heart disease will
experience an atherosclerotic changes – the changes that occur
because of the build up of plaques or fatty streaks on the interior
walls of the blood vessels that supply oxygenated blood to the heart
muscle. This condition is also known as
coronary
artery disease.
In particular,
when the arteries become clogged with deposits made up of "bad"
cholesterol,
plaque, scar tissue, or calcium, the heart has a harder time
circulating blood. This clogging causes a myriad of heart problems
from
angina
pectoris (chest pain) to heart failure to a
heart attack.
The World Health
Report 2002 estimates that over 3% of all disease burden in
developed countries is caused by physical inactivity, and that over
20% of coronary heart disease and 10% of stroke in developed
countries is due to physical inactivity.

Major Types of Heart
Disease:
There are
three major types of heart disease:
1.
Coronary artery
disease (CAD)
is the most common type and is the leading cause of heart attacks.
When one has CAD, the arteries become hard and narrow. Blood has a
hard time getting to the heart, so the heart does not get all the
blood it needs. CAD can lead to:
a.
Angina. Angina
is chest pain or discomfort that happens when the heart does not get
enough blood. It may feel like a pressing or squeezing pain, often
in the chest, but sometimes the pain is in the shoulders, arms,
neck, jaw, or back. It can also feel like indigestion (upset
stomach). Angina is not a heart attack, but having angina means you
are more likely to have a heart attack.
b.
Heart attack.
A heart attack occurs when an artery is severely or completely
blocked, and the heart does not get the blood it needs for more than
20 minutes.
2.
Heart failure
occurs when the heart is not able to pump blood through the body as
well as it should. This means that other organs, which normally get
blood from the heart, do not get enough blood. It doesn’t necessary
mean that the heart stops. Signs of heart failure include:
a.
Shortness of breath (feeling like you can't get enough air)
b.
Swelling in feet, ankles, and legs
c.
Extreme tiredness
3.
Heart arrhythmias
are changes in the beat of the heart. Most people have felt dizzy,
faint, out of breath or had chest pains at one time. These changes
in heartbeat are, for most people, harmless. As one gets older, it
more likely to have arrhythmias.

Diagnosis of Heart
Disease:
The diagnosis of heart disease is based upon clinical
history and physical exam. Confirmatory tests include:
-
Electrocardiography
(electrodes are connected to the chest and heart activity is
monitored) and measurement of the level of serum creatine kinase
enzymes released into the blood by the damaged muscle.

Aniography
(a heart imaging technique) which injects dye into the arteries
followed by x-ray.

What are the symptoms of Heart Disease?
In the early stages
of heart disease, there are no symptoms. The first symptom is
usually
angina
pectoris (chest pain) or
heart attack.
Coronary artery
disease (Angina)
can be described as
a discomfort, heaviness, pressure, aching, burning, fullness,
squeezing or painful feeling in your chest. It can be mistaken for
indigestion or heartburn. Angina is usually felt in the chest, but
may also be felt in the shoulders, arms, neck, throat, jaw or back.
Other symptoms that
can occur with coronary artery disease include:

Shortness of
breath
-
Palpitations
(irregular heart beats, skipped beats or a "flip-flop" feeling
in your chest)
-
A faster
heartbeat
-
Weakness or
dizziness
-
Nausea
-
Sweating
Heart Attack
(Myocardial Infarction or MI)
Symptoms can include:
-
Discomfort,
pressure, heaviness, or pain in the chest, arm or below the
breastbone
-
Discomfort
radiating to the back, jaw, throat or arm
-
Fullness,
indigestion or choking feeling (may feel like heartburn)
-
Sweating,
nausea, vomiting or dizziness
-
Extreme
weakness, anxiety or shortness of breath
-
Rapid or
irregular heartbeats
-
During a heart
attack, symptoms typically last 30 minutes or longer and are not
relieved by rest or oral medications (medications taken by
mouth). Initial symptoms can start as a mild discomfort that
progresses to significant pain.
Some people have a
heart attack without having any symptoms (a "silent" MI). A silent
MI can occur among all people, though it occurs more often among
diabetics.

What treatments are available for Heart Disease?
1.
Medications
a.
Beta-blockers
lower blood pressure by reducing the amount of blood pumped by the
heart. Calcium channel blockers relax blood vessel walls, thereby
lowering pressure.
b.
Diuretics
lower blood pressure by causing the body to expel excess fluids and
sodium through urination.
c.
Angiotensin-Converting Enzyme (ACE) Inhibitors
expand blood vessels and decrease resistance.
d.
Statins are
very effective in lowering LDL ("bad") cholesterol levels and have
few short-term side effects.
2.
Balloon Angioplasty - A nonsurgical procedure designed to dilate (widen or expand) narrowed
coronary arteries. The result of this procedure is that the blood
vessel is dilated, and blood can flow more easily through the
(formerly narrowed) part of the coronary artery.
3.
Bypass graft surgery is to construct a new channel so blood can get around the
atherosclerotic blockages in the coronary arteries. Therefore,
instead of trying to scrape out the plaques, the surgeon uses a
segment of a vessel from another part of the body to transport blood
to the far side of the obstruction. Usually the grafts are fashioned
from one of the large, accessible saphenous veins that run down the
inside of the leg, although recently there has been a trend towards
using the internal mammary arteries located under the chest wall.
